HONESTY IS A MUST

• Owners MUST provide their valid identity number (NIK/Passport)
• Owners MUST provide complete information of their pets

HEALTH

• Current Vaccinations/Veterinarian Information - Owners must verify their pets are up to date with following vaccinations;
  Dogs: Canine Parvovirus, Canine Distemper Virus Leptospirosis and Infectious Canine Hepatitis, Canine Rabies Virus, Canin Bordetella Virus.
  Cats: Rhinotracheitis Virus, Calicivirus, Panleukopenia Virus, Chlamydia Virus.
  Proof of Vaccination shall be provided to on request as well as current Veterinarian information.


• Health or Medical Problems & Elderly Pets - Some procedures sometimes be uncomfortable for your pets especially for a senior pet or pet with health problems, and can expose hidden medical problems or aggravate a current one during or after the procedures. In the best interest of your pet, this contract/agreement will give us permission to obtain immediate Veterinary treatment for your pet should it be deemed necessary by us


• Fleas/Ticks - LUKS PET GROOMING & SERVICES is a flea/tick-free environment. When fleas/ticks are found on your pet during the body screening, we reserve the right to proceed with ‘The fleas/tick removal procedure’ and an additional charge will incur on your bill. Pets with a flea or tick infestation carry a risk of health problems. We recommend that you contact your veterinarian for advice on the best treatment for your situation.


• Infectious Skin Diseases - Healthy skin and coat are every owners’ dream for their pet, even though some infectious skin diseases may not be seen or known on daily basis. When a light infectious skin disease is found on your pet during the body screening, we reserve the right to proceed with the ‘medicated bathe treatment’ and an additional charge will incur on your bill. When the infectious skin diseases are not in our capacity to treat, we recommend that you contact your veterinarian for advice on the best treatment for your situation.

BEHAVIOUR

• Well-Trained - When your pet is not qualified to a well-trained behavior standard, a leash or a pet carrier is a MUST when entering LUKS PET GROOMING & SERVICES’ premises.


• Aggressive or Dangerous Pets - Owners MUST inform us if your pet bites, has bitten or is aggressive to people, other pets or specific procedures. We reserve the right to refuse or stop services of your pet at any time before or during the process and charge an Aggressive Pet Fee of IDR 100k in addition to the regular charge. If your pet should bite or scratch, the owner agrees to be responsible for any and all related medical bills, recovery costs, loss of income and equipment damage.

MEDICAL ATTENTION

• There is always the possibility an incident could occur. We will do our best to contact you first, then take your pet to our authorized Veterinarian, or to the nearest Veterinarian that is available. It is agreed that all expenses for Veterinary care will be covered by the pet’s owner.

CHECK-IN / CHECK-OUT

• Proper ID is required to check-in and check-out your dog. Dogs will only be released to the owner or to persons identified prior to pick up by the owner via written and signed documentation.

APPOINTMENTS

• A 15-minute late without any confirmation within 1 hour of your appointment is consider a No Show/Missed Appointment.
• Rescheduling is only available once (at least one day before the appointment). We reserve the right to put your new appointment based on our availability.

PRICING

• All Prices Assume a Reasonable Condition and Temperament.
• Cancelation within 24 hours of your appointment a 50% charge will incur.
• No Show/Missed Appointment is nonrefundable.

EXTRA

• Every pet is unique. Any special request that require extra services, time or product involved regarding your pet’s habit has to be informed during the booking process and additional charges will incur on your bill.
• When your pet require medication, you will need to supply the medication. Medication need to be clearly marked with pet’s name, instruction and dose.

BILLING

• All BILLS have to be settled UPFRONT

LATE POLICY

• When owners failed to pick up their pet within 15 minutes after the scheduled time or our message/call confirmation, an additional ‘Late Fee’ of 50k/hour will incur on your bill.

UNCLAIMED PET

• Please be a responsible pet owner. When owners did not claim their pet without any proper arrangement or payment within 48 hours after the scheduled time or our message/call confirmation, we reserve the right to proceed with any possible solution.

GROOMING

• Matted Coats - LUKS PET GROOMING & SERVICES provides removing small knots, tangles and isolated matted hair from the coat which can be achieved quickly without causing anxiety, stress or pain. The health and condition of a dog’s coat and skin are the responsibility of the owner (along with its physical and psychological state of health). Professional grooming plays an important role in the commitment undertaken when choosing any breed of dog. This level of commitment is evident in the condition of the coat due to effective home visit and temperament due to socialization and training.


• Accidents - Grooming equipment is sharp, even though we use extreme caution and care in all situations. Possible problems that could occur include cuts, nicks, scratches and quickening of the nails. In most cases this can happen when a dog is wiggling or moving around. We make every effort to ensure your dog is groomed as safely as possible, but when your dog does not accept the process it can be dangerous to continue the groom.

BOARDING

• Diet - We apply a ‘One Meal A Day’ diet to all stay-in dogs in order to keep them healthy. Dogs that are fed one meal per day appear to have better cognitive function and health than those fed multiple times.


• Activity - All stay-in dogs are entitled for free playground access. They have to be back into the boarding room for the rest of the day.


• Behavior - Dog with high aggression behavior or in heat signs has to remain in the boarding room.


• LUKS Bathe - Is a mandatory when your dog wants to stay with us. All monthly stay-in dogs are entitled for a free LUKS Bathe session.

PLAYGROUND

• Behavior - Dog with high aggression behavior or in heat signs is not allowed in the playground.


• Day care - Day care service is available from 09.00 – 17.00 WITA.
